I too am JCV Positive and taking Tecfidera but they will do blood work every 3 months to closely monitor your lymphocytes.
They check your JCV titer as well .
I do worry about it even though the risk is very very low but I get my JCV antibody levels checked every 3 to 6 months. I am not sure how lymphocyte levels have anything to do with PML but JCV antibody levels do for sure.
Even people who test negative for JCV are not in the clear. It could be a false negative and you can become positive at any time including the day after your blood test.
I get Tysabri infused every 6 weeks because the latest research shows that can reduce PML risk significantly, and this helps me deal with the risk A LOT better.
My neurologist doesn't talk numbers but if I test positive for JC virus I'll stop Tysabri.
I wouldn't want to live with the fear of PML.
I get tested every three months for JC virus.

Although, I tested negative as a carrier, I am still pretty concerned and worried about the numbers and risk for the infection. My doctor only checks numbers every six months. At my appointment next month I am going to request a script to have it checked once a month. I feel its best to stay on top of it myself as doctors these days don't seem to. I just started Tecfidera last week.
