Can they tell by MRI if your MS is secondary progressive. Mine have been very stable last five years with no activity and walking fatigue has become more challenging the last year. But MRI's show no activity. MS since 1998. Which I am thankful no active lesions. Any thought warriors!!!
My brain and spinal cord MRIs did not show any changes but I sure had them. My doc manually tested me and looked at what changes I was having and realized I had moved to secondary progressive. That was two years ago. Just had a cervical MRI this month and it showed changes neurologically and structurally. So I'm at physical therapy now trying to limit the shocks my GP thinks are coming from my cervical changes. Doing a lot of stretching and some light weights. Neuro doc is next week. In the meantime I breathe deep walk meditate eat nutrient dense foods and all-around have as many happy moments as I can create.
Well, as far as progression goes, it is just based on Your history and Your Dr's observations.
I myself have had no New activation, but The past scares have shown no repairs anymore.
Now, part of this Dx is because I have had MS for 26 years. I was not changed in Dx until year 23, so I count myself lucky. But, everyone goes into a progressive status after a period of time, so don't feel bad.
I beleive the only reason it took so long to change My status was due to good living and Medical advancements since Dx. I was also on study with Avonex for the first 15 years as well, so take that as You may.
Positive attitude is everything in the battle as well. So keep it to up. I know We all will see the End to MS one day, soon hopefully. Stay Positive...
Ciao for Now.

My neurologist changed my diagnosis from Relapse Remitting to Secondary Progressive because my walking was what he called spastic. He timed me and has been watching me for several months. He hasn't taken me off the medicine, but I will ask him when I see him in May 2016
No. The dx changes to progressive only after a lot of observation. My neuro believes that I'm probably secondary because I don't really get a break, but there are no meds if he makes it official. Beyond that, I don't know about how it pertains to lesions, etc. Have you talked to your neuro about meds, supplements, and therapy? I hope you have a wonderful birthday.
@A MyMSTeam Member he doesn't feel it has gone secondary I don't think. We he mentioned waiting for new drug and I said it is going to be for secondary FDA approval first....he said he would mark it RR/SP?
When he timed walk a few weeks ago best ever. I said not really accurate because if I keep walking I fatigue out. So trying to change up my routine and go walk and exercise in the pool along with my weight training and yoga. Feels it is damage from lessons in early years. No black holes and that of mine is normal for my age. All positive. Didn't know could tell by spinal fluid. Never had one when first diagnosed. Also didn't realize structural differences with MRI's. Thanks bestie at MS knowledge.
