Please confirm for clarification purposes if I understand of your question is correctly ? You are not working, you're collecting "Long Term Disability" benefits from you employer and your employer has extended you health benefits for 6 months. So now you want to know what happens after the 6 months when extended health benefits cease ? As well as what happens it you're approved for SSI Benefits ?
Being that you're already on Long Term Disability the insurance company should have assigned one of its in-attorneys to assist you through the maze of filing for SSDI benefits. Reason being it's to the insurance company to help get you approved as it reduces it's financial liability regarding benefits paid to you. The benefit currently being paid to you by the insurance company gets reduced by the amount paid by SSDI benefits if approved by SSA. The SSDI benefit is not in addition to what you're currently receiving on Long Term Disability.
It's important to understand that having been approved by for Long Term Disability Benefits does not mean that you are "entitled" to SSDI Benefits nor does having MS "entitle you to SSDI benefits. You have to prove to the SSA that you no longer are able do the work you were trained even with "reasonable accommodations" being made and that you're not trainable for another position even with "reasonable accommodations made. In fact I know people that are receiving Long Term Disability Benefits that have MS an have been denied SSDI benefits even having gone through a full court hearing.
Yes I got rejected at First, then I went to them and introduced myself and then I explained my and gave them all my Paperwork and then a couple weeks later I got a didn't answer. Plus I had a Witness to Explain why it's better for me to cut back on my hours and wham I worked Part-Time then it was Ridiculously how my Employer got Ridiculously Stupid so I just Quit by just walking Out. I just moved my 401k and changed my Healthcare and I never went back.

I have a great neurologist who documents very well. I also have copies of all the reports since I filed short term disability. Unum will probably assist me but from what I've read online I would need to repay them when I'm approved. The payback is the difference between SSD awarded amount and Unum payment that I would receive on long term disability.
Unum is my companies insurance disability provider. Their approval rating is above most. Yes, thank you I looked at the forms. I'm waiting to hear from them next week.
