I have SPMS, diagnosed a couple years back. Now I get "relapses" that are not true relapses. All the symptoms (worse fatigue, worse pain, decreased function, etc) but no new lesions on the MRIs. After a bit, symptoms get better, but I never fully return to level of function I was at before the episode. Over time, just a steady overall decline that never gets any better. I am 61. So much for my golden years. ๐Ÿฅด
