It represents a phase where symptoms gradually worsen over time, leading to increased disability. In contrast, PPMS (Primary Progressive MS) is progressive from the very beginning. People with PPMS never experience periods of remission - their symptoms continue to worsen from the start without the relapsing-remitting pattern that precedes SPMS.
