Oh gosh, its the worst isnt it. The pain in the jaw, temple and face can be unbearable at times.
I was prescribed carbamazapine for it but it didn't have much effect. I wish I had a magic remedy for you, if you find one in the meantime please let me know π

What are the best options for managing trigeminal neuralgia (TN) when anticonvulsants and gabapentin aren't fully controlling the pain?
First, raw snakes are off the menu β but there are some real options worth exploring with your neurologist. Since talking triggers pain and it's your livelihood, getting this under better control is genuinely urgent. Here are some approaches that may help beyond what you're already taking:
Medication Adjustments
- Adding a muscle relaxant like baclofen alongside your current medications may boost their effectiveness
- Pregabalin (Lyrica) or lamotrigine (Lamictal) are alternatives worth discussing if not already tried
- Topical options like Voltaren gel applied along the cheekbone have helped some MyMSTeam members
Procedural Options
- Gamma knife radiosurgery is commonly used for MS-related TN β it's considered safe and effective, though pain relief may not be permanent for everyone
- Microvascular decompression (the Jannetta procedure) places padding between a blood vessel and the trigeminal nerve, if compression is the cause
Mental Health Support
- TN with MS significantly increases anxiety and depression risk. A psychotherapist specializing in chronic pain can help manage the emotional toll, especially when it's affecting your work. Since your ability to speak is directly impacted, it's worth having an urgent, focused conversation with your neurologist about procedural options. Medication alone may not be enough, and there are surgical paths that could offer more meaningful relief.
