Smoldering MS is a continuous process where MS gets worse even during periods when you're feeling well. It's characterized by slowly expanding lesions in the brain that remain active and continue to grow over many years, causing damage to nerve cells in affected areas.

These lesions, also called rimmed lesions or chronic active lesions, have distinctive dark boundaries that can be seen on specialized MRI scans. People who have more of these rimmed lesions tend to experience worse mobility and cognitive issues at a younger age, even when on MS treatment. Several lifestyle factors can affect smoldering MS:
• Lack of exercise
• Poor diet
• Smoking
• Excessive alcohol consumption
• Poor sleep habits

The progression from smoldering MS can be subtle and difficult to notice during regular doctor visits. While there aren't specific symptoms associated with smoldering MS, it can contribute to gradual disability progression even during remission periods. It's important to discuss any subtle changes you notice with your neurologist to help monitor your condition.
