I would assume it is bad especially if it is smoked. There is an actress named Selma Blair and she uses nicotine patches as an off label way to help with her ms fatigue . Patients with Parkinson’s do it also.

How does nicotine affect MS?
Nicotine has several harmful effects on MS. It compromises the blood-brain barrier, which is already vulnerable in people with MS. This can potentially worsen the condition's impact on your central nervous system.
Nicotine also affects your immune system, which is particularly concerning since MS is an autoimmune condition. The chemical may trigger or worsen the overactive immune response that destroys myelin in MS.
Additionally, nicotine is found in cigarettes alongside other harmful chemicals like cyanide that have demyelinating effects - meaning they can destroy the protective myelin coating on nerves, similar to what MS does naturally.
Because of these effects, nicotine-replacement products like patches, gum, or e-cigarettes may not be a healthier option for managing MS, even though they're sometimes used for smoking cessation. It's important to discuss any nicotine-based quit-smoking strategies with your doctor to understand the potential risks specific to your situation.
