I have been strength training since my DEXA 1 1/2 years ago revealed my osteoporosis. My lastest DEXA scan showed worsening T-scores and now my PCP wants to put me on a drug treatment.

There are quite a few options. Your doctor can probably let you know which medication will work best for you

I take Alendronate Sodium once a week. My osteoporosis improved to osteopenia, I'm not sure how to spell it.

It's really encouraging that you've been committed to strength training — that dedication matters! It's really encouraging that you've been committed to strength training — that dedication matters! Unfortunately, for some people with MS, exercise alone isn't always enough to stop bone loss, especially when MS-related inflammation, reduced mobility, or medications are also affecting bone density.

Many MyMSTeam members have faced a similar crossroads. One shared, "I have been on most therapies, including monthly steroids. I now have osteoporosis and the steroids can have an effect on your bones." Another noted how difficult it can be to keep up with weight-bearing exercise alongside MS symptoms. When lifestyle changes aren't enough, medication is a common next step. Options your PCP might consider include:

- Bisphosphonates (like Fosamax, Boniva, or Actonel) — typically the first-line treatment for slowing bone loss
- Denosumab — a biologic sometimes preferred for certain patients
- Other options like Tymlos, Evenity, or Forteo, which can help rebuild bone mass

These come in pill, injection, or IV infusion forms, so there's some flexibility. It's also worth discussing vitamin D and calcium levels with your doctor, as both are essential for bone health and MS can make deficiency more likely. A registered dietitian can also help tailor an eating plan to support your bones alongside your MS management.

The good news is that medication combined with your existing strength training routine could be a powerful combination — you're already doing the hard part!
