That's a really important question, especially when managing both RRMS and possible PPMS. Unfortunately, there is currently no single blood test that can reliably predict or confirm an MS relapse (flare-up) is starting.

MS activity is typically monitored through a combination of tools, including:

- MRI scans – to detect new or enlarging lesions in the brain or spinal cord
- Neurological exams – to track changes in function or symptoms
- Cerebrospinal fluid (spinal tap) – which can show markers of inflammation like oligoclonal bands

Some emerging research is looking at blood-based biomarkers, such as neurofilament light chain (NfL), which may indicate nerve damage and disease activity. However, this is not yet a standard routine test. The best step is to talk with a neurologist about any new or worsening symptoms as soon as possible. With PPMS, symptoms tend to progress gradually rather than in clear relapses, which can make it trickier to identify a flare-up versus general progression.

Keeping a symptom journal can be really helpful to share with your care team so they can spot patterns and act quickly if needed.
