"Nerve damage and pain: Direct demyelination can cause neuropathic back pain that manifests as stiffness, often described as a burning or aching tightness.
• Secondary musculoskeletal issues: MS symptoms like weakness, balance problems, or altered gait can lead to poor posture, compensatory muscle strain, or joint tightness in the back, exacerbating stiffness. "

Can new, unexplainable back stiffness lasting over a week be caused by MS?

Yes, back stiffness can be related to MS. Between 10 percent and 16 percent of people with MS experience back pain, and stiffness is a common symptom. Many MyMSTeam members have shared similar experiences with back stiffness that developed without an obvious injury.

MS can cause back stiffness in several ways:

- Muscle weakness and poor posture: MS can weaken muscles and affect coordination, leading to posture changes that cause stiffness and pain
- Spasticity: Muscle tightness, cramping, and stiffness are common MS symptoms, often worse at night
- Immobility: MS-related fatigue, dizziness, or mobility issues can make it harder to stay active, contributing to stiffness
- Compensatory movements: Altering how you walk or move to accommodate other MS symptoms can strain your back muscles

One MyMSTeam member described waking up with stiff leg muscles and body, walking bent over in the morning before improving after a few hours. Another shared experiencing "horrible stiff shoulders and neck" with muscles that felt like they were pulsating.

However, back stiffness can also have other causes unrelated to MS, including age-related changes, arthritis, or muscle strain. It's important to contact your healthcare provider about this new symptom so they can evaluate what's causing it and recommend appropriate treatment.
